What to do if a work injury occurs
  • The employee IMMEDIATELY reports the injury to the employer.
  • If the work injury is emergent, the employer directs the employee to the nearest medical facility for emergency treatment.
  • If the work injury is not emergent, the employer may refer to WorkStar’s provider network, HealthSmart. If additional assistance is needed, please contact WorkStar at 1-800-256-8833.
  • Within 24 hours of the injury, the employer or the injured worker should contact WorkStar Health Services at 1-800-256-8833 or 513-779-5680 to complete a First Report of Injury. A FROI (Download > FROI in English or FROI en Español) may also be completed and faxed to WorkStar at 1-800-256-9540 or 513-779-5681. You may also complete a FROI online at the Ohio BWC website.

Managed Care Organizations partner with the Ohio BWC to medically manage worker’s comp claims and help control worker’s compensation costs. If you are a new Ohio employer and do not select an MCO, one will be randomly assigned to you.
