WorkStar Health Services for Providers

We at WorkStar Health Services are committed to working collaboratively with the providers who care for our injured workers. Our focus and goal is a safe and timely return to work. We utilize Official Disability Guidelines as guidelines for treatment and return to work timeframes.

Filing a Worker’s Comp Claim
  • The employee is encouraged to IMMEDIATELY report the injury to the employer.
  • If the work injury is emergent, the employer directs the employee to the nearest medical facility for emergency treatment.
  • If the work injury is not emergent, the employer may refer to WorkStar’s provider network, HealthSmart. If additional assistance is needed, please contact WorkStar at 1-800-256-8833.
  • Within 24 hours of the injury, the employer or the injured worker should contact WorkStar Health Services at 1-800-256-8833 or 513-779-5680 to complete a First Report of Injury. A FROI (Download > FROI in English or FROI en Español) may also be completed and faxed to WorkStar at 1-800-256-9540 or 513-779-5681. You may also complete a FROI online at the Ohio BWC website.

Requests for treatment

Any medical treatment that falls outside of the BWC’s presumptive approval guidelines must be requested on a C9 form. If you have any questions about the need for a C9 form please contact WorkStar Health Services at 1-800-256-8833 or 513-779-5680.

Compensation Benefits:
The Bureau of Workers’ Compensation handles all compensation benefits. For additional assistance, you may direct the injured worker to call 1-800-OHIOBWC.
